If an output is over loaded and fails does it fail "ON " or "OFF "?

Some one correct me if Im wrong but an out put will fail in the off position, there is a chance that a relay module may fail on but it would be vary rare.
Do you have a life safety setup you are concerned about?

I 've seen DC (transistor) outputs fail either way (open or shorted). That's why we wire some limit switches to kill the movement independent of the PLC execution if continued motion could cause damage.

We do something similar to what Bernie mentioned. We group the outputs so that movements using multiple outputs share the same common. We then route the common through a limit switch, safety relay, etc en route to the output card.
I usually end up with several different output common "types ". Some are switched with the e-stop circuit, some go through additional safety devices, some (like alarm horns, beacons, etc) are hot any time there is control power.
