P1-04ADL-2 not reading input voltage

I have this module on a P1-412. 24v applied to the module. 5v applied to channel 1, and I get nothing. I tried all 4 channels and same thing.

Data view shows under range error on all 4 channels. Module failed bit is off. Missing 24v bit is off.

 

Am I missing something or did I get a DOA module?

    Just tested this setup and working as expected. Getting 4127 counts with a 5 vdc input on channel 1. Call into our RA dept to get a replacement.

    I was guessing that would be the case. I tried the same setup on a Click I had here and it worked fine. Thank You

    Just to verify is your 5V and 24V referencing the same common? If the 5V is floating the input card wouldn't be able to read it.

    Yes, the voltages were coming from a 5/12/24vdc power supply with the same common
